::Doctoral Student Forms Needed (In Order)::
1) A Program of Study (POS) must be filed within the first year. Three committee members are needed for this form and one must be from another department.
2) The Request for Preliminary Examination may be taken after most/all courses have been completed, but not later than the semester after all coursework is completed.
No form is needed to schedule a dissertation proposal.
3) Should the student add/drop a course or a committee member retire, then a Change of Committee/Program form must be filed.

4) The Approval to Schedule Doctoral Defense is completed when the student is ready to defend the dissertation. Dr. Talab arranges this defense.
5) For students who live outside of Kansas, an additional form must be filed, Request for Videoconference/Teleconference Final Defense form.
Dr. Talab also arranges for this form and it is submitted along with the Approval to Schedule Doctoral Defense form.

::Master's Student Forms (In Order)::

1) A Program of Study (POS) must be filed within the first year. Two committee members are needed for this form. Courses should not be older than 6 years at the time of graduation.
2) Should the student add/drop a course or a committee member retire, then a Change of Committee/Program form must be filed.
3) The Request for Final Oral Examination from must be sumitted and the final oral exam is scheduled by Dr. Talab. The student presents the final e-portfolio and technology project to the committee.

::Certificate Student Forms (In Order)::
1) Student submits a Certificate Application form. Courses should not be older than 6 years at the time of graduation.
2) Once the final e-portfolio url has been approved by Dr. Talab then a Certificate completion form is submitted by Dr. Talab to the Graduate School verifying that all Certificate courses have been completed. The Graduate School then executes the Certificate and sends it back to Dr. Talab to send to the student.
